Whoa! That’s a lot of forms! These forms are required and State mandated. Your child is important to us. You can download (click on the text of the description) and fill these forms in or you are welcome to pick up a folder in our Preschool Office. (Email Form 1 and Form 2 to preschool.fumc@gmail.com. This will officially enroll your child and will secure the opening while you work on the rest of the folder.)

2. ACH Tuition Form – this form allows us to automatically draft your bank account for monthly tuition. Drafts are made on the 1st of each month. You will have access to ProCare Tuition Management System to manage your account.Download
3. Folder Checklist – this is a checklist of all the forms needed below and will be attached to folder if you could print this first.Download
4. Emergency Card – this form is used in the classroom for emergency situations. It is also used in a potential offsite emergency situation, so please think carefully and fill it in completely.Download
5. Physician’s Report – this form needs to be filled out by your child’s physician. A TB test is not required.Download
6. Health History – this form gives us a more in depth look at your child; their fears, allergies and anything we can do to make preschool a wonderful experience for them.Download
7. California School Immunization Record – fill in the top portion of this form and bring your child’s immunization record with it and we will fill it in.Download
8. Consent for Emergency Medical Treatment – this form allows us to treat your child if they get injured at preschool. Download
9. Identification and Emergency Information – this form is an extended emergency form that is kept in the office.Download
10. Personal Rights – this form shows that your child has rights at our school and you are welcome to see them at any time.Download
11. Notification of Parent’s Rights – this form states your parent’s rights at our Preschool. Return only the signed portion.Download
12. Directory and field trip permission – this form allows us to add your name to our family directory to arrange playdates or get togethers. It is not mass distributed. The form also gives permission to go on school field trips. Download
13. Digital and Photography Permission – this form allows the preschool to take pictures of your child for art projects and our webpage. No names are ever used. It also allows the Preschool to text and email you important events.Download
